Question About Submission Form to Use with Voucher

A while back I renewed my membership and have a voucher for 15 free submissions. I went to the PSA website to print off a submission form, but in red type it states: "This is not to be used for complimentary submissions."
The printed submission forms I received from PSA in my membership packet look exactly like the forms that can be printed off the website. Beause they both are alike, it didn't make sense to me that I could use one and not the other. Can anyone please tell me if there is a special submission form for the membership vouchers? Thanks in advance for any input.

They send you a submission form that states it is for such subs.
15? sounds like your free subs may have expired.
That was changed. When did you sign up?
If it was more than a year ago, call customer service on Tuesday.

I can't find this anywhere... I just used the online form to submit my 15-card membership voucher and had no problems.
Right, the years changed from 2 to 1 but the 15 vouchers stayed the same. (Platinum)
When ever I re- up my membership, I get a packet and in that packet is a voucher giving me X amt of free grades
AND a submission form for the free grades.
Not sure what the problem is. If the OP somehow mis placed the form all he has to do is call customer service.
He still must have the voucher which IIRC is numbered.

When using the free vouchers from the Set Registry Benefits, I do use the sub sheets they send me because they have specific writing on them made for that particular voucher.
