Submission Form with More Lines

Does it exist? I don't want to break my submission into 3 small lots because there weren't enough lines -- can we turn in custom submission forms?

I know there is a website to add numerous entries - personally I just fill out an excel spreadsheet with the columns they have on the form, and say "see attached" on the actual submission form where would enter card info. Then print the spreadsheet out and staple to the form, but be sure to include the submission form number in case they get separated, and your customer ID and email. Never had a problem.
