When do you stop and question that a holder has been tampered with?

When buying a card over the internet when do you really start to question if the case has been tampered with?
1. Do you think Every case might be altered before buying?
2. If you just see a little frosting?
3. You just feel its over graded?
4. When buying a card under 100.00?
5. When buying a card from 100-500?
6. When buying a card 500.00+?
7. Or just because of who is selling it?
8. Other(explain)?

Thanks to some of the pessimists (or is that realists??) on here, I seem to be questioning every holder over $50 unless I know the seller.
