Apologies for the poor scan, as it was done within a scratched lucite holder. But I know you've been asking me for a few months...can't say as I recall the last time I saw a similar example.

Yes, they don't come up often. I've only seen 2 in the past year. One on eBay and one private sale. There are actually 2 versions, I don't have either.
