Help with sending in more than 15 cards.

I am about to send in my 1st order that won't fit on 1 order form. I found the link to print pages that show 25 additional cards per page but is there anything else I need to do when I send in the order? How do I list the final total declared value? Do I need to make extra copies of the additional pages and send them in? Any advice would be most welcome.

Edit to add: you don't have to list the total declared value anywhere. The declared value is just used to calculate shipping costs.

They put my order on Receive Hold when I forgot to put the declare value.
