It's not a uncommon as you would expect, especially with older cards.
I am an absolutely smoking Norm VanBrocklin Bowman rookie that is NM-MT to MINT, but PSA keeps putting it in a PSA 5. I stopped cracking it out and sent it in for review and the Post-it note pointed to the very fine embedded hair and identified it as a "surface crease." I've known all along that it's there, but I've had other cards with the same defect holdered in PSA 8's and will expect the same on this. I left the Post-it on there and added one of my own, "Loupe it and you'll see it's not a crease, but a fine hair or fiber embedded in the gloss." We'll see how that turns out...
