Ilarione is a City in Italy, it's actually called San Giovanni Ilarione. I think that is what the S.Ilarione stands for... the ABR (ABE) and Prot.DI parts I have no clue... I think the last part may be reference to Catalonia??? Hard to read... Thats about all I can come up with.. Your family Italian?

There were a few Saint Ilariones, so it may not be describing a town. "Abb" is short for abbazia = abbey. It is definitely Italian, using "di". But I can't make out that last word? and what is written under 1903?

Most of the religious medallions I'm personally familiar with are more modern (grandmother came over in the 50s) so I can only speak very generally on this one - the ones I've seen usually have a more purely religious motif. This looks like the sort of design I'd expect from a layman. That's just me thinking out loud on this - I guess I'm just saying it's different
