1962 Jefferson Nickel in a PCGS MS65 Holder. I bought this 8-10 years ago on Teletrade. It was brilliant when I bought it and now it has a hazy crud on both sides. What would cause this to happen- PVC?

I think you would be better off just buying a few bank wrapped rolls of 1962 Jeffersons for about $3/roll and save yourself much hassle with PCGS.

Depends on what was paid for the coin at the time it was purchased... as long as the buyer has a receipt, a full refund is possible. If the buyer paid lots of money for the coin, and bought it off TT, he should be easily able to get a receipt.
If it's a $10 coin, then forget it and move on.
