Its a 42 mm silver annual medal of Pius VIII. Year 18 featuring the restoration of the Laocoon group on the reverse.
Die cracks are pretty common on medals and many of the earlier papal/italian states pieces but very fun nonetheless. In my experience, doubling is especially common on Naples/Two Sicilies coinage of the 19th century. In the case of my medal, the two marks on the obverse in the left field of my medal were, in fact, made by fairly large die cracks indicating a late striking of this medal. Gotta love the coinage and medals of this period!
