Recessed detail missing

I'm curious about the possible causes of missing recessed detail on a coin. For example, I have seen some Standing Liberty quarters that are missing the deepest feathers on the eagle's wing. Normally, the high points are vulnerable to wear or incomplete striking.

Would incomplete "low point" detail be the result of die wear or die polishing?

    Yes, I thought of the possibility of some kind of debris or obstruction in the die. The result is a smooth and somewhat polished looking part of the coin.
