A Submission Questions

I'm submitting 20 cards so I have to fill out 2 submission forms......Do I have to pay the shipping/handling/ Insurance on both submissions?

Sorry for the elementary question but this is only my second submission.

Are they both for the same submission? If no, then yes you need to pay 2 return shipping fees. If they are under the same sub, then no, they will grade and ship at the same time.

Only 15 cards fit on a sub form so I had to use a second form for the additional 5 cards. Do I just cross out the sub # on the second form and write in the sub # from the first form on it? Or would I have to use a second submission form for the additional 5 cards?

Thank you very much for those forms. I've always used my own "mickey mouse" version of a continuation form. It was crude and a tad on the sloppy side.
