Psa/Dna Question.................

I recently had a Quick Opinion done for a E-bay item and it came back "likely Genuine" and I won the auction. Psa/Dna's website states they will refund you the $7.49 for the quick opinion if I send the item in to have it physically authenticated. What do I have to send in with the submission to receive the $7.49 Credit ?? Anyone Know ??

Print out the "Likely Genuine" email - they're really good about that
