How do I fill out a submission form for a mechanical error?

I'm getting ready to send in a card that is labeled wrong. How do I fill out the submission form? I the only cost to have this done the shipping costs? Thanks in advance.

Fill it out as if you were sending in a regular sub, Put the Mech Error on a sub form by itself and write in big letters across the top of the form "MECHANICAL ERROR"

In regards to your question on fees and costs. I received this in response about a month ago from PSA - which I believe would answer all your questions on the same topic.


If your card is labeled incorrectly you would need to submit it as a mechanical error submission. Below are some instructions for sending them back in to be fixed. These instructions are very specific to insure that we are able to process your order properly and avoid delays.

· Please fill out a submission form, mark "Other" as the service level, write in "Mechanical Error" and include a short note on the form describing the error. There is no charge or return shipping fees for the correction.

· If you are not a Collectors Club member, please provide me your address and I will send you a submission form.

· Please keep the problem item(s) on a separate submission form if you will be sending other items in to be graded.

· Please make sure the submission form and the outside of the shipping box has “Mech. Error" clearly and LARGELY written somewhere on it.

· You will still need to write in the Declared Value(s) and sign the submission form.

· Once the error is corrected, you may contact us for a grading voucher to compensate you for your shipping costs. This voucher will arrive separate from your order.

You can either send this in or wait for the show to fix the label. We would most likely be able to do it at the show for you free of charge.
