Problems with PCGS grading/labeling

Has anyone ever received a coin back from PCGS with one grade, but their online database shows a different grade ?

Happened to me recently. Jefferson Hag coin came back in a MS69 holder, but my submission form shows the coin in a MS70 holder.

    I recently sent 5 coins in for crossover at PCGS. Out of the 3 that crossed, they sent back one coin labeled wrong and another coin with someone's hair in the holder. The one they labeled wrong was supposed to be a 1905 U.S. Philippines One Centavo, but it came back labeled as a 1905 IHC (wrong coin #, but they had the right grade). So, I called customer service and they told me to send them back and indicate MECHANICAL ERROR on the submission form.

    The worst for me was that I got my coins and the numbers never got entered into the system. I ended up having to scan both sides of each slab and e-mail them to customer service.

    To add insult to injury they mislabeled 4 coins (omitting first strike designation). I too had to send them back indicating on the invoice Mechanical error. That was almost a month ago, and I still haven't received my coins back.
