Altered Coins, cleaning, etc.

So, as you guys may remember, I've started collecting again after a 15 year hiatus or so. So, back in the good ole' days, we had lots of issues with differing grade opinions (and PCGS was just starting to take off). However, I don't remember there being a ton of concern around altered or cleaned coins. Is this a fairly new phenomenon? If so, how prevelant is it?

    It has always been a problem. Seems to be more prevelant and the methods more sophisticated---e.g. lasering hairlines on proof gold.

    greed follows the money; big bucks are being paid for toned coins. So lots of coins doctors creating AT's to try to get the NT money.
