VAM Help Needed: 1891-O Triple/Double Stars

It doesn't seem to match the VAM 12. That's the only one I can see that mentions double stars.

All right stars are doubled. All left stars are doubled or tripled. Small die crack through neck to '1'.

Any ideas?
