Error experts please help!

I have a 96 Lincoln, philly mint, that has a flatened head from the eyes all the way to the top of the head and part of the In God We Trust. the rim is normal and the reverse is as well. it looks like the flatest strike I have ever seen, but could possibly be some sort of error. does anybody know what could make the coin look like this? Are the dies clogged or worn?

    It's hard to attribute an error without seeing it in person but it sounds like you have a filled die, in which the recesses of the die was filled with debris, usually strike hardened grease, dirt and gunk. Value is around $10-$15.
