Quarterly special question

I know I've seen the instructions in a post before but its late and cant find it.

What has to be written on the submission form and on the submission box?


With tomorrow being the deadline (for all practical purposes) I dont want to get my submission wrong.
I'll see your bunny with a pancake on his head and raise you a Siamese cat with a miniature pumpkin on his head.

You wouldn't believe how long it took to get him to sit still for this.

    I am assuming that the $12.45 Total Declared Value for return shipping still applies correct?

    just woke up and getting ready to head to the post office. I know that the $5.00 handling fee doesnt apply, but does the return shipping and handling of $12.45 apply? I assume it does but I have never done a quarterly special before and I want to make sure I have the paperwork filled out right.

    I would assume that on the submission form you would check "Other" and then write in CC Special.

    I decided to change calling the bathroom the John and renamed it the Jim. I feel so much better saying I went to the Jim this morning.
