What do you guy's write on the submission form and what's the service cost?

It depends on the service level selected.
Anyone know the cost if the coin is holdered? Is it $15 for the re-holder and $5 for the photo?

$5 and $5 I believe. A total of $10 if it is already holdered.

Anyone else have other details?
And coins that are sent in to be crossed are not eligible for Trueview.
