Need the extension of the submission form. Please help.

I'm looking to send in approx. 75 card and I would like to get them all on one submission. I saw an extension of the submission form on here some time ago. Can somebody help me out with this?

i saved that link, that would have cam in handy in june with the 5.00 special with my 300 cards lol,i got it for next time anyways!
