BGS submission...

I had a post a couple of weeks ago about submitting to BGS with this free certificate that I have. I got on there and was looking at the form. I haven't really done this before and need some help with it. Is it better to fill out the online form or send a paper form in with your submission. Also, this return postage stuff. Do I have to send a box with my submission for them to ship my stuff back to me? Just curious. Any help would be appreciated. Thanks!
