2006 W $10 Platinum American Eagle Proof 1/10 oz graded PR69.
Comes with a set of the US Mint packaging. I've got two of these, and when I sent them off for grading I did not mark the packaging so I could reunite the coins with their respective packaging. Hopefully that isn't too important.
