Oddball Question: Carol Wright Proofs?

Can anyone help me identify this issue? They show up on eBay fairly regularly, but some sellers call them Carol Wright Proofs, while others call the JKA Button Proofs.

Any help?

    Wow I dunno about this Carol Wright is/was one of those fake junque sellers back in the day-you know the stuff you didn't know you needed until you saw it in the catalog-jeez she did cards too? OMG
