GEM, I've lived in Nevada for over 50 years and have never seen that happen to a quarter or half. Other than in shop class! Slots don't damage coins. As a collector for most of that time I believe that I would have spotted that in one or more of the count rooms I've worked in, alas I have not.
We did mutilate a lot of coins in shop class at Carson High as I recall.

Even in his pictures of the "smooth edge" you can see traces of the reeding in the top coppernickel layer to the left and right of center. (Dead center they are obscured by the "hot spot" of the lighting)
