It was $10 per ticket, any year(s). I don't recall if there was a minimum or not but I sent in 20 tickets. Considering that Vintage tickets (1979 and earlier) normally cost $25, I could not pass it up. The modern tickets are $15. I am surprised more people did not take advantage of it.

No, you don't. I give the significance if any such as Ripken's 3000th hit. Personally, I would rather not see the score and pitcher since most of mine have something to do with a milestone. I only give the information that I am interested in, and PSA adds what they want.

Those are pretty cool, I've got a couple from last years opening day Cardinals game that I was thinking about sending in. They are just a little different size/shape than standard tickets, do they do those?
