Is this the correct attribution of this coin?
Which country is it from? (Poland?)
Is it listed in Krause? If so, what's the KM number?
Also, what is the approximate value?

    Looks like a Bohemian (currently Czech Republic) Gross, should be about as large as a quarter or slightly larger, and really thin.

    Jester's right, it is Bohemian. And you're right, it is Vladislaus II. I've seen them labeled as "Prager Groschen" as far as denomination goes.

    The strike weakness is very typical so value is probably $50.00-$75.00 or so. Full strikes bring $100+
