Three Different 2006 D Nickels?

Look at these three nickels, the normal one would be to the far right. What's wrong with the others? Three Nickels

Possible different die stages...

Pillars on top of the stairs appear to be missing

the stairs on the first nickel are missing all together. And what is that on the second ?
