White Spots on American Silver Eagles

Dear fellow collectors:
Recently I had purchased quite a few American Silver Eagles on ebay and many of them come with white spots on them even though they were graded by either PCGS or NCG. I would like to ask for your expert opinions on the following questions:
(1) Do you see white spots on the ASE you purchased or own? What percentage (have spots)? What do you do with them (return it or?)?
(2) Do you know what cause this problem? (I even see this happens to the 2006W 20th Anniversary Silver Ealge and I think they were made with the special burnished planchet).
(3) Do you know if PCGS or NCG would buy back coins graded by them that have problems? (*I read in another thread that a collector bought a PCGS graded U.S. gold coin and later thumb print show up on the coin. He called PCGS and they told him to resubmit the coin for reveiw and PCGS would either pay for the price difference if the coin is downgraded or buy it back from him.) Can somebody elaborate on this policy because I certainly couldn't find this policy in the PCGS web site and I had troubles in getting replies from PCGS.
