What form to use for Complimentary submissions

Can anyone tell me what form to use for my 6 Complimentary submissions? As far as I can tell, the normal form you can download, is not to be used for these.

There should have been a form, card savers and stickers in the package you received when you joined. I'm getting ready to send in my 6 for renewing my membership.
