My face still turns red every time I recall my initial bulk grade painful learning experience - thought I could grade SBAs and submitted over 40 80-S and was so confident I wrote "NO MINIMUM" on the form:
got back 40 nice plastic holders with MS64 and 65s inside, not much fun I can tell you. Rob
Modern dollars are like children - before you know it they'll be all grown up.....
The no decision/refund on the 1916 dime was likely a very worn coin being sent in for a 1916-D, but the mintmark area was just too worn out. I have a wannabe 13-S quarter in now with a clear S, but the date is very worn. Might was well to see if I get lucky on this one, even a PO-1 is ma valuable coin!

This was a really weird one. I renewed my membership at a Long Beach show and just handed over 8 ANACS slabs I brought to sell to dealers to use the certificate before I would lose it. I forget the grades but they are all high MS VAMs (and some DMPLs) and I probably specified the same grade as the minimum. Shouldn't do things like that. Oh well. There is not another submission I have done that is that bad.

It was my first submission (my collector's club submission). Before I realy knew what I was doing. I got two BB morgans for cleaning and a questionable color 1909 VDB. I still dont think one of the morgans was cleaned and after seeing other early red Lincolns my VDB definately had questionable color. It looked more orange than red. Probably treated with a weak acid. That is as bad as I have had.
I did later have an Isabella that I crakced out of a ANACS MS62 slab. PCGS BBed it for cleaning. I sent it back to ANACS and they put it back in a MS62 slab. Its funny how TPG companys differ as to what is market acceptable.
3 for 8 on my first attempt at cross overs which were all NGC slabbed coins. All early date merc dimes. One that did cross, which had crossed from a ANACS slab to a NGC slab earlier, made up for the whole mess as it went up a grade and also picked up the FB designation. So on this coin that had been sent to the big three only PCGS said it had full bands. It was and is FB all the way IMO.
The last couple of Merc submissions have been Raw or Crack Outs. No problems with them.
I sent a 100 coin modern proof order in and they sat for 3 months before grading and they hazed. Mostly came back pr 66-67 no cam. I still have some of them--------BigE
3 for 3 BB. This was my first submission---coins I had bought years earlier. Didn't understand the BB concept at the time--expected net grading. Afterward I recalled the dealer mentioning to me that the dollar had been cleaned. I've gotten several opinions that the IHC should grade. What think ye?
SOME of my worst submissions consist of Lincoln Memorial cents when they are graded mostly MS-64RD, sprinkled with (oh my, how thoughtful, PCGS!) an occasional MS-65RD.
What do I do with these kinds of submissions? They're stuck in their PCGS boxes and set aside w/o further attention.
My first submission was the worst. 20 ANACS to be crossed. 14 bodybags. Since then, I have learned to be more careful. I chalked it up to the cost of education.
ANACS, who slabs everything in sight, bodybagged 2 coins. >>
They only bodybag for PVC or counterfeits. Why were yours BBed? All other problem coins get a net grade.

A lot of times it's a coin with no obvious problems, but worn so much that they can not determine the year or M/M.

It's also sad and disappointing to see that what some TPGs think is market-acceptable often actually isn't, except at a deep discount.
