Two PCGS Mistakes for the Price of One. Well... Maybe...

Ok, on This Kennedy Half Dollar the insert states, Filled Die Obv. No "FG"
Now, I'm thinking, the "FG" (designer's intitials) are the on reverse and not the obverse, like later issues within the series. Those initials are missing on this coin because they were polished off the die. As far as errors go, this one is fairly common (unlike the 1966 with a missing FG).
So, did PCGS miss that and believe the die was filled (possibly with grease) leaving off the "FG" on this struck coin, or is the obverse really struck from a filled die (I can't see the evidence in the photo) and the coin also is the "FG" error variety?
I'm puzzled.
