For some reason this is a fairly common afliction for '72-S quarters. It is seen to a much lesser extent on the '71 and even less on the '73. It's not terribly uncommon on the '68-'70 quarters either. I suspect that for some reason a press was used far beyond the point at which proof dies are normally swapped. This may have happened more than once in '72 and '68.
Some of these will be a lot worse than the pictured coin.

MadMarty was posting while I was typing. It is worse than any I've ever seen as well but would have predicted it would be a '68 quarter.
