I'd grade the picture PR68 Cam AH if that's lint on the holder and there are no hairlines. 67 or lower if otherwise. The tick on the ear and the stuff at 12:00 on the reverse appear to be on the coin, and the dark spot on the cheek is probably inconsequential in hand under normal light. Pretty spot free for an AH.
