Extra Long Submission Form

I know I've seen it posted previously, but I wasn't able to find it in a search - would someone mind posting the link to the extra long (longer than 25 lines) PSA submission form. I assume PSA has no issue with this form (if anyone's had a negative experience, please let me know), and it sure beats printing out multiple forms. Thanks in advance.
