It can be done in many ways. This one was done by using an absulutely beautiful Heritage reverse image
as the background. (Unfortunately you have to sign in to see it.) The Antietam reverse makes for a beautiful canvas.

Instead of using a beautiful large scale coin image as the canvas, try overlapping many related icons over each other.
This montage was created by picking nice images, slicing out the icons, reducing the opacity of the original coin, then
layering the icon back over the original coin.
