PSA Flip Errors

It looks like someone fell asleep at the wheel. I understand that mistakes happen. However, I am a little worried when I send in multiple copies of the same card for grading. For example, if I send in five Roger Clemens rookie cards for grading and I get three - 10's, one - 9 and one - 8 is there a chance the PSA 8 could end up in the holder with a PSA 10 flip? And then I am left to ponder why my card with PSA 8 flip looks mint.

Hmm ... The card does not have a black border and the signature is not a facsimile. It is one I obtained in person and sent into PSA/DNA. Looks like I'll be sending this one back.
