When my scanner is back online, I will post a scan of a 1900 S Morgan Dollar I have that is very worn. However, this coin has what appears to be a chip/crack on the rim. Are chips (a piece of the coin falls off) part of a faulty minting process?

It would depend. You could be describing what is known as a lamination error that has been bent back too far on itself and broken....hard to tell without pics.
