mechanical error submission question

I've got a stack of cards to return to be labeled/coded properly. Do I need to fill out the form as if they were raw cards -- i.e. list everything as it should be? Should I tape a post it to each slab with with the correction should be?
What has worked in the past for anyone who hs done this?

~"I apologize for the inconvenience. However, if you will send the (1) card back to PSA, the error will be corrected. These directions are very specific to insure that I am able to follow the order properly, otherwise it will cause a delay. Please fill out a PSA submission form, mark "other" as the "service level", write in "mechanical error" and include a short note on the form describing the error. Please include a copy of this email to help expedite the order. Please keep the problem card on its own submission form. Please make sure the submission form and the outside of the shipping box has "ATTN: " clearly and LARGELY written somewhere on it, you will still need to write in the “declared value” for each item as well as the total "declared value". I will take it from there. If you are not a Collectors Club member, please give me your address and I will send you a form. PSA handles many cards on a daily basis, for this reason it is very important that the submission form is filled out completely and legibly. Once the card is corrected due to “mechanical errors,” you will be sent a grading voucher to compensate you for the shipping back to PSA, this voucher will arrive separate from your order. Again, I apologize and thank you for your patience."~
At the time it was attention to Juliet Martinez but she is no longer in charge of that area.
