Evolution of PSA labels

Does anyone know how many labels have been used, and when? I have examples of 3:

1: White part has curved edges on front, "PSA" hollogram on back (my example is a 030 number)
2: White part has curved edges on front, "Collectors Universe/The Collectors Company" hollogram on back (063 #, 1999 card)
3: White part has squared edges on front, "A divison... CLCT" on back (all others I have)

    Here is my guess:

    1st was the flip with the zero with the slashes in them.

    2nd is the hologram one with the psa in it.

    3rd is the hologram with collectors universe.

    4th is the one with no hologram, light blue backing.

    5th is current with no hologram and darker blue backing.


    Now I am not sure on the holograms which one came first, that is just a guess.
