I have a bunch of 60's proof and mint sets and none of them are cameo. So my question is what causes it to be a cameo? Is it a weak die?

It's actually the opposite; a strong die. The cameos in the earlier years were the first coins produced from freshly prepared dies. As the dies were used the cameo gradually fades as the dies become worn until it's completely gone and the coins become brilliant. The very early strikes are what produced deep cameo coins.
At the time there was no deliberate attempt to produce deep cameos as there is today.

A ton. The vast majority of the material that's on the market has been picked over. Add to this the fact that there weren't that many produced to begin with, and it becomes very difficult to find the keepers. Plus, everybody and his brother is hunting so when something does show up it gets snatched up fast.
