Scans of tampered with PSA holders?

So my next question for you all is from the barely visible to the obvious, can anyone share scans of holders that have been tampered with (for card switch outs or otherwise)? I ask so that the less experienced among us (myself foremost) might be able to know what to look out for as I begin changing over my buying habits from ungraded to graded cards. This seems to be the only possible risk when buying graded cards as you've told me and it would be nice to make sure it's not something that can go completely unnoticed.

    With scans [and photoshop] - there really isn't much to learn. If the holder is cracked - or one side looks really, really cloudy, those seem to be the biggest warning signs to me.

    But you have to rely on dealer integrity, etc. when you are buying based on a scan.
