Scans of tampered with PSA holders?

So my next question for you all is from the barely visible to the obvious, can anyone share scans of holders that have been tampered with (for card switch outs or otherwise)? I ask so that the less experienced among us (myself foremost) might be able to know what to look out for as I begin changing over my buying habits from ungraded to graded cards. This seems to be the only possible risk when buying graded cards as you've told me and it would be nice to make sure it's not something that can go completely unnoticed.

If there is clouding around the edge, the holder is likely tampered with. Also, if there is any question in your mind that the holder looks different, stay away. There will always be another one to be found soon.
