PSA7 Orr Rookie in a tampered slab (Final resolution)

This is in reference to the thread I made regarding a PSA 7 Bobby Orr rookie in which the slab turned out to be tampered with. PayPal contacted me today and it looks like they've pressured the seller in granting me a full refund. A quick check of my PP account verified that the funds have indeed been transfered to my account. I now have a balance for full amount I paid. It was real pain and I had to FAX a scanned receipt of the seller's signature on receiving the item before they could put the final nail in his head, but at least it's finally over.
The guy should have worked with me. I would have gotten the card regraded for him for free. Now he's going to have to do it (and pay for it) himself.

You bought a card from Macattaq who bought it from RedZone.
You thought the card had a crease and sent it to PSA for them to look at it.
PSA looked at it and determined the flip was switched, and the holder was tampered.
They cracked it, and sent you the raw card back.
You sent the raw card to Macattaq, and you got your refund.
Is that what happened?
Questions:
How was the labele switched? Is it obvious when you held it in person? I'm guessing not since you didn't suspect that when you sent it to PSA. What are the exact issues that PSA saw - I know they said something about milky areas on the sides, but frankly, I have some holders that are milky in some places (not much, but still there)...does that mean there is a chance those were tampered with? I'm mostly worried that if someone can tamper and switch labels, that buyers (me especially!) can get screwed!
