What form do you use for you 10 free submissions?

I went to down load the submission form online and it say

This is not to be used for complimentary submissions.

What form do I use???

    The form should have been sent to you when you signed up.

    I still have that one. it is the carbon copy one with four sheets. Correct?

    Yep! It should already have a $0.00 in the grading fee section.
