Need fact sheet for a 1975 Bahamas proof set

Anybody have a 1975 proof set for the Bahamas?
I need a copy of the fact sheet description, front and back, that is on the page titled something like "Proof Set Information" or "The 1975 Proof Coinage of Bahamas."
I'd appreciate a copy if anyone has one.
