WHERE TO GET THE SUBMISSION FORM?

I am not a member of the Collectors Club. ...........

......I do however, have various sets in the PSA set registry, including one which is 75% complete. I emailed for and recieved a code for one FREE submission for the set.

Where do i find the submission form??

Collector of:Baseball
1955 Bowman Raw complete with 90% Ex-NR or better

Now seeking 1949 Eureka Sportstamps...NM condition
Working on '78 Autographed set now 99.9% complete -
Working on '89 Topps autoed set now complete
