Extra Submission Forms (for large submissions)

Hi all,

There was a recent thread about how folks submit large quantities when the online submission form only has 25 lines on it. Hopefully PSA will fix this someday and allow members to enter the number of cards they are submitting and the submission form will automatically generate the correct number of lines.

Until then, I have worked up a few forms that will accomodate 50, 75 or 100 card submissions. Feel free to use them and let me know if anything needs to be added or changed to make them better. (I may add a place for Account # and Name, just in case the pages get seperated). Let me know what you guys think.

If they look strangely familiar, I (uh hem) "modified" the one we all know so well. Hopefully PSA won't have a problem with me "borrowing" from their original. If they do, I'm sure I will hear from them.

    If anything, PSA should give you some free subs for taking care of these forms for your
    fellow collectors. They are way overdue on making forms like you have!!

    Larry - thanks, very nice helpful modifications! Appreciate the efforts!

    As a suggestion, being able to put your name and customer number on each form would be good - as would forms for up to 200 cards. I always wondered why PSA never made the online submission form easier for larger submissions, especially when everything else they do on-line is top notch.

    Looking for 81-84 Topps Stickers in PSA 9 or better, 81 Topps Scratch offs, 83 Topps Fold outs in PSA 8 or better, 83 Fleer Stamps and 81/86 Fleer Star Stickers in PSA 9 or better.

  • Hi all, an update on the forms:

    - I added "Account #" and "Name" to the top of the forms.

    - I made the form slightly shorter to ensure it prints on only one page. Is it printing okay for everyone now?

    - I added some "Baseball only" submission forms with the SPORT column pre-filled and the QTY column set at 1. (Forms for "Football only" coming sometime when I get some time).

    I was wondering if it was possible to modify the forms to go up to 250 cards? I submit 250 at a time and it would sure save me time if I could use these forms.

  • Hi all and thanks for the support, comments and suggestions for these. I hope everyone is making use of them. Here's an update:

    I've added "Football ONLY" forms and now there are forms for up to 150 cards.

    More to come as I get the time.

    Thanks for sharing. Personally, I created an excel spreadsheet that I simply attach to the submission for. I simply write "See attached sheet" and haven't had any problems.

    I have the extra forms saved and just used them for a 88 card 1978 Topps submission! They are a great tool!

    for my last submission, i used an excel spreadsheet and then cut/paste the submission header and footer. it printed out very well, so i'm hoping there are no issues with it.

    Just used again.....

    maybe someone can revamp these and make the numbering match up with the current submission sheets.

    my PSA sheets end with line 20, not line 24.

    use the Extra submission forms with the online forms. The numbering works fine than.
