Everything is now the same from day one, except for as Iseeyou stated, the MC miscut qualifier was later added.

Sometimes cards are returned as miscut, meaning the angles of the cards are not a perfect square. These cards look more like a parellogram. I dont think they would fit in the holder. They will grade cards so far cut, another is showing, provided they are square.

If the corner of the picture (diamond cut to the extreme) is touching the side edge, or is cut off at the edge, PSA will NOT grade it. They will return it to you as miscut. In the mid to late 1990's when a card was miscut less than what was afore mentioned. PSA still didn't grade it. They sent it back as miscut. I would guess that many people must have complained about this since they still had to pay the grading fee, while SGC and other card companies would encapsulate thes lightly miscut cards, they just gave them a lower grade. They must have decided to ease the complaints by created the MC qualifier.
Someone mentioned that the OC qualifier is the least used. You are right to a degree. It used to be used the most befor but it seems that PSA has gotten a bit leanient on the OC qualifier, from what I have seen the past couple of years. You have to remember that PSA is a business looking for profit. Angry customer don't make them that much profit.
For those who might have believed Ylder, a (MC) qualifier means the card is the proper size and shape, but the card is so far off center that part of another card's picture is on the card (often recognized because part of that card's picture is missing). If a card isn't the proper shape, but PSA doesn't conclude that it was trimmed after it left the factory, it will be rejected as a Miscut - it will not be graded, and a submission voucher for a future free grade will be given.
