What caused this on the coin? Is it similar to milk spots or is it some form of toning?

It could be that the coin used to be toned and was dipped. Removing toning causes fine pitting on the surfaces. It could also be milk spots.

I see that kind of toning all the time on silver eagles still in the tube that froze due to improper storage. Not sure the reaction there but I've seen it.

Well, the pic makes it look more extreme than it probably is. Given the grade, I'd guess it's just toned white.

I see that type of uneven/splotchy toning/appearance on Peace Dollarts quite frequently.

I believe it is often the result of toning which has eaten into the metal slightly. Even if dipped, such a coin will typically display areas of surface damage from that toning.

No expert here, but it looks like a dip that wasn't properly neutralized.
