Submitting World Coins to PCGS

I have reviewed all the literature that came with my PCGS "Collector's Club" membership but still don't know a few things. Can anyone tell me, please:
1): What is the distinction between the "World Rarities" and the "World Coin" submission categories? Is it strictly determined by the coin's value, or are there other criteria?
2): Can I submit both gold and silver together on the same submission form?
3): What does one enter under the column for "PSGS Coin No." for a world coin?
